Combine all ingredients (except the ketchup) in a large bowl. Mix together with your hands until well blended. Place meat mixture in a 9x16 baking dish. Bake for 40 minutes or until meat is cooked thoroughly (I like the edges a little burnt). Remove from oven and spread the ketchup on the top. Replace back in the oven and continue cooking for 10 more minutes.
