Here is how you make Rachel's Meatloaf:

Heat oven to 375 degrees. Combine all ingredients (except the ketchup) in a large bowl. Mix together with your hands until well blended. Place meat mixture in a 9x16 baking dish. Bake for 40 minutes or until meat is cooked thoroughly (I like the edges a little burnt). Remove from oven and spread the ketchup on the top.
