Here is how you make Loaded Potato Salad:

Cut the potatoes into approximately 3/4 inch cubes. Place in a pot of water and bring to a boil and cook until the cubes are soft but not mushy. Drain. Add the rest of the ingredients and mix well but don't overhandle so that the potatoes lose their shape. There should still be chunks of potato visible. Put into serving dish and chill until served.
